Our program in the East Valley area is currently seeking to fill Full-Time Bilingual LPN position.
SCHEDULE:
Depending on program and client need. Some nights, some weekends. This position is full time, anywhere from 30-40 hours per week.
DUTIES AND RESPONSIBILITIES:
Document (verbal/written) and submit significant and routine paperwork such as progress notes, statistics reports, daily activities, upcoming events, special concerns, varied incident reports, etc., as required by the program. Complete all necessary documentation, to include the following: intake/psychosocial assessments, treatment plans, treatment plan summaries/monthly case staffing (to be completed during staffing), weekly case staffing, discharge summaries. Distribute and monitor client medications. Maintain a tracking system for all nursing documentation and maintain files. Work with program administrative staff to assure client files meet standards. Complete IR/MIR/Infection Control reporting needed during shift, as appropriate. Prepare for and participate in monthly psychiatric evaluations. Complete health assessments to include pain screening, nutritional screening and growth charts for each client within appropriate time frame. Complete monthly vital checks.

3. Basic computer skills required.
4. Arizona LPN License
5. Must have clean 39 month Motor Vehicle Record (No tickets, moving violations, etc)
6. Must be at least 21 years of age (25 to drive 15-passenger vans).
7. A valid Arizona Drivers License
8. Current and valid level 1 DPS Fingerprint Clearance card. A New Leaf may assist you with your fingerprinting renewal process upon expiration of current DPS Fingerprint Clearance Card.
9. Spanish-bilingual is a requirement for this position.
