Direct Support and Rehabilitation Specialist

Primary Functions: Coordinates and monitors services for the member and family as outlined on the individual service plan developed by the Child and Family Team/Adult Recovery Team for those individuals who have been determined to be high risk and in need of more intensive supports to maintain in their home and community.  Works as a member of the CFT/ART Team to assess strengths, needs progress and goals.  Assist CFT/ART Team with clerical, outreach and coordination activities identified in the consumer and family/support service plan.  Engages in community outreach to develop resources to enhance and support service delivery capacity.   Assures accurate communication with community, team members and fellow SEABHS employees. Provides services throughout the various SEABHS locations as needed.  Reports to their assigned Clinical/Program Supervisor.

 This position is established to serve as part of a team of support for consumers and their families and to provide creative services to youth and families to help support youth in remaining in their homes and communities.  It exists as a key role for the purpose of fulfilling The Arizona Vision, which means:

Minimum Qualifications: At least one year of experience in behavioral health or one year of experience providing structured recreation services.  Must possess a valid Arizona Driver's License.  Must become certified in First Aid, CPR and CPI, within six (6) months of hire.  Must be able to obtain Fingerprint Clearance.  Must be 21 years of age or older.  This position requires availability to include nights, weekends, holidays.


  • Cognitive and willingness to learn.
  • Knowledge of functional assessment and positive behavioral supports; ability to provide rehab and support services to youth and families to assist them in maintaining in their homes and communities
  • Ability to maintain good consumer relationships, possesses excellent verbal and written communication skills, and be able to identify priorities.
  • Ability to maintain confidentiality of all client matters and handle organization confidential matters with discretion.
  • Demonstrated willingness and ability to project professional demeanor when representing the agency during community assignments or within collegial relationships within the agency.
  • Basic computer skills.
  • Demonstrate good teamwork skills and team player attitudes.


Team Participation

  • Participates in Adult Recovery Team/Child and Family Team meetings of the members involved in the high need programs as well as for those members being referred to the services.
  • Provides support/rehabilitation activities, health promotion, living skills services, pre-vocational and vocational services as determined by the Adult Recovery Team/Child and Family Team and specified in the Individual Service Plan. Services may be provided 24 hours per day, 7 days per week so flexibility and availability are important
  • Provide transportation for high need/high risk members to assure presence at appointments as determined appropriate and medically necessary by the Adult Recovery Team/Child and Family Team
  • Assists in strengthening/building continuum of care of services including informal supports for the Adult Recovery Team/Child and Family Team.

 Community Development

  • Builds relationships and collaborations with community-based partner agencies that enhance the service delivery system.
  • Ensures continuous outreach and engagement efforts to identify persons with behavioral health disorders and promote public awareness of treatment facilities available.
  • Collaborate with other SEABHS staff to identify opportunities for increasing community awareness of behavioral health programs, improving access to care for community members and referrals from collaborating agencies.
  • Represents SEABHS in an appropriate and positive manner when participating in community forums or committees as well as when meeting with stakeholders and partner agency staff.

 Program Implementation and Service Delivery

  • In collaboration with the assigned Clinical/Program Supervisor, develop programs designed to improve life skills and community functioning of members including goals and objectives, activities, procedure manuals and outcome measurements.
  • Implement and deliver programs to referred members that build teamwork, improve communication skills, and enhance interpersonal relationship skills in the person.
  • Coordinate and schedule activities for program participants to assure availability of positive recreational and skill building opportunities for referred members.
  • Coordinate or provide transportation for program activities.
  • Encourage and facilitate participation by family members in member activities.
  • Knowledge of eligibility status when providing services.
  • Prepare and maintain well written clinical records accurately and thoroughly as required ethically and legally.
  • Assist, as requested, with planning and implementing like programs at other SEABHS locations.
  • Submit all required documentation requests by payroll, personnel, and provide any funding agency reports on a daily, weekly or monthly basis, as required.
  • Participate in clinical supervision in accordance with ADHS Licensure/Contracts as assigned.
  • Be familiar with and comply with all relevant agency policies, state and federal statutes and rules.


  • Perform rapid drug screen via urine specimen, as needed. Staff member must be properly trained on urine collection, as required by the Rapid Drug Screen Lab Test policy
  • Performs other duties, as assigned.
