Jobing Description

Primary Functions: Provides support and rehab services to adult members to assist in maintaining housing and living independently.

Minimum Qualifications: At least one year of experience in behavioral health or one year of experience providing structured recreation services.  Must possess a valid Arizona Driver's License.  Must become certified in First Aid, CPR and CPI, within six (6) months of hire.  Must be able to obtain Fingerprint Clearance.  Must be 21 years of age or older.  This position requires availability to include nights, weekends, holidays.


  • Cognitive and willingness to learn.
  • Knowledge of functional assessment and positive behavioral supports; ability to provide rehab and support services to enrolled members to assist them in maintaining in their homes and communities
  • Ability to maintain good consumer relationships, possesses excellent verbal and written communication skills, and be able to identify priorities.
  • Ability to maintain confidentiality of all client matters and handle organization confidential matters with discretion.
  • Demonstrated willingness and ability to project professional demeanor when representing the agency during community assignments or within collegial relationships within the agency.
  • Basic computer skills.
  • Demonstrate good teamwork skills and team player attitudes.


Team Participation

  • Participates in Adult Recovery Team meetings.
  • Provides support/rehabilitation activities, health promotion, living skills services as determined by the Adult Recovery Team and specified in the Individual Service Plan. Services may be provided 24 hours per day, 7 days per week so flexibility and availability are important
  • Assists in strengthening/building continuum of care of services including informal supports for the Adult Recovery Team.

Community Development

  • Builds relationships and collaborations with community-based partner agencies that enhance the service delivery system.
  • Represents SEABHS in an appropriate and positive manner when participating in community forums or committees as well as when meeting with stakeholders and partner agency staff.

Program Implementation and Service Delivery

  • Knowledge of eligibility status when providing services.
  • Prepare and maintain well written clinical records accurately and thoroughly as required ethically and legally.
  • Submit all required documentation requests by payroll, personnel, and provide any funding agency reports on a daily, weekly or monthly basis, as required.
  • Participate in clinical supervision in accordance with ADHS Licensure/Contracts as assigned.
  • Be familiar with and comply with all relevant agency policies, state and federal statutes and rules.
  • Performs other duties, as assigned.

Physical Requirements:

  • Use a Personal Computer and/or telephone up to five hours a day
  • Frequent long, irregular hours
  • Frequent and/or prolonged travel
  • Occasional reaching, bending, stooping, kneeling or crouching
  • Frequent standing or moving throughout facility and/or outside
  • Participation in games and activities with the clients (i.e. basketball, softball, walking, running)
  • Light physical effort (lift/carry up to 25 pounds)
  • Ability to operate a motor vehicle
