Jobing Description
NOTE: Must have current C.N.A. certification in Arizona.

STATEMENT OF WORK:
After training and demonstrated ability, the home health aide/C.N.A. provides personal care and other assigned therapeutic activities for clients on an intermittent basis and under the supervision of an RN. Tasks are according to a plan of care and specific and individualized for each client; these tasks allow the client to have his physical and emotional needs met and remain in his home environment. The home health aide/C.N.A. may also perform any other job related tasks deemed necessary and assigned by the supervisor.

DUTIES & RESPONSIBILITIES
Prescribed:
• Assists with personal care including bathing, skin care, hair care, oral hygiene, and nail care, as well as shaving and toileting.
• Monitors and documents vital signs and reports any variances to the supervising nurse.
• Assists with catheter care, bowel, bladder or ostomy program.
• Assists with range of motion and other simple exercises as delineated in the aide care plan.
• Assists the client with the application of special appliances and /or prosthetic devices.
• Assists the client with transfers to and from the wheelchair and bed. This includes the use of the Hoyer lift or other mechanical devices.
• Cleans and straightens the client's bathroom and bed area after care is rendered.
• Assists the client with simple meal preparation, offers water and refills water at the client's bedside.
• Reports any significant change in client condition or negative or unusual occurences to the supervising nurse.
• Accurately documents care given and submits paperwork to the office according to company policies.
• Meets company standards for productivity.
• Reminds the client about medications that need to be taken.

Varied:
• The home health aide/C.N.A. is an integral member of the health care team and provides services to the clicnt that allow the client to remain in his home; the aide/C.N.A. encourages the client in self care as much as possible and reinforces teaching that the home health aide/C.N.A. and client have been instructed in previously.

KNOWLEDGE, SKILLS & ABILITIES
Minimum Required:
• Must have high school diploma or GED.
• Must have current C.N.A. certification in Arizona.
• Must have fingerprint clearance.
• Must complete 12 hours of inservice training yearly with the calendar year.
• Must have a car, car insurance, and be willing to drive to see clients in their homes.
• Must have current CPR training.
• Must have first aid training.
• Must be able to document care legibly and submit paperwork timely.
• Must meet company standards for productivity.
• Must attend case conferences and home health aide/C.N.A. meetings routinely.
• Must be able to stoop and bend and physically assist the client with his needs, be able to lift 50 pounds, and have no restrictions for physical work.

Preferred:
• One year experience working in the medical field with the elderly.

WORK EXPERIENCE & EDUCATION
Minimum required:
• Must be able to work independently, have reliable transportation, and enjoy working with the elderly. Must be able to read and write legibly and have reliable transportation, car insurance.

Preferred:
• A skilled worker with an affinity for working with the elderly and vulnerable of our community.

WORKING CONDITIONS:
• The majority of the work time is spent working in clients' homes and traveling to assignments.

COMMENTS:
• The Home Health Aide/C.N.A. is a skilled individual who is sensitive to the needs of the client, alert to changes that must be reported, and makes a significant impact in the independence of the clients.
