The primary responsibility of the Client Care Coordinator is to assist the RN Clinical Manager and Care Coordinators to ensure that clients are staffed with qualified personnel. The Client Care Coordinator works closely with RN Clinical Manager and Care Coordinators in the field to ensure that all client intakes are handled professionally and timely to coordinate the needs of the client. Works closely with the Data Entry/Billing Specialists to ensure accuracy of intake data. Acts as a non-clinical liaison between the RN Clinical Manager, Care Coordinators, Case Manager, client, family members and other field staff. Coordinates and reports all non-clinical and scheduling information to the involved team in a timely manner. Acts as a facilitator between referral source and clients, Case Managers and family to ensure client specifications are communicated with all involved persons. Assists the RN Clinical Manager and the Care Coordinator with the staffing coordination of care of all Home Health clients. Reports directly to the RN Clinical Manager.

Skills / Requirements
  • High School Diploma required with preferable college courses in business and/or secretarial skills. Prior experience working in a Healthcare facility as a Client Care Coordinator preferred.
  • Knowledge of A/R billing included but not limited to UB92, HCFA 1500, HCPC coding and CPT coding.
  • Ability to work closely with insurance companies, referral sources, clients and insurance case managers.
  • Excellent Customer Service and communication skills including professional, courteous telephone skills.
  • Empathetic attitude towards the care of the ill and elderly patient.
  • Has an understanding of each service offered and how to access these services.
  • Knowledge of Federal, State and Local regulations regarding home health care.
