STATEMENT OF WORK:
Home Health experience is a must. Also must have excellent computer skills and have good working knowledge of OASIS, Medicare regulations regarding home health care and excellent computer skills. Responsible for staff management and coordination of the agency clients as well as the documentation and maintainence of the client records. This includes the oversight of the supervisors, their function in both the office and the field and the supervision of in house and field staff. Must have excellent knowledge of documentation requirements including Federal, State and local regulations. This includes the responsibility and oversight of QI function of the agency and knowledge of OASIS reporting and significance of reports. Excellent knowledge of Medicare home health requirements is a must as this RN is involved in the day to day operations including chart review and assignment of staff to best meet client need as well as computer data entry and tracking. The RN Clinical Manager works well with others both within the agency and in the community. Excellent computer skills a must as well. DUTIES & RESPONSIBILITIES Prescribed: • Oversight of staff assignment by the supervisors for client care for the day to day management of the clients. • Assists the supervisors in the assignment of appropriate fill in staff when regular field staff absences are necessary. • Ensures quality service is being provided to all clients of the home health agency. • Ensures that field staff are being properly supervised. • Must work well with varied staff and serve as a resource for questions from both field and office staff. • Communicates with staff regarding written documentation involving client files, of contacts with case managers, physicians, staff in the field. Serves as backup for the supervisors and troubleshoots issues. • Oversight and knowledge of the cases that the supervisors open for service. • Education of the supervisors and staff in the creation of the plans of treatment for the clients . • Inputs POT information when necessary for the Medicare/ Medicaid clients and works with staff nurses to ensure that the POT reflects care to be given. • Educates staff in the ongoing changes to the OASIS documentation. • Oversight of the OASIS data input for the assigned clients. • Performs reviews of client files for accuracy and adherence to the plan of care. • Acts as leader of the QI team. • Reviews forms for consistency, and makes changes as appropriate. Oversight of all of the client care teams. • Audits the client records to ensure compliance with regulations. • Works with staff to meet deadlines regarding all aspects of the agency. Must be accustomed to paperwork deadlines and have success in meeting them. • Acts as a resource for the supervisors and provides assistance to the field staff as well when necesssary. • Works with the Administrator to update policies and procedures as needed. • Works with the Administrator to see that budget goals for agency are met. • Acts as the alternate Administrator in the absence of the Administrator of the home health agency for whatever reason. • Ensures compliance with local, state, and Federal regulations. Varied: • Works closely with the Administrator and the Supervisors for the coordination of the varied services of the home health agency. • Serves as the leader in QA activities in the agency; involved in all aspects of the QA process and attends and participates in monthly committee and other activities. • Other tasks deemed necessary by the Administrator. Skills / Requirements
KNOWLEDGE, SKILLS & ABILITIES
Minimum Required: • Must be Arizona licensed RN. • Must have strong supervisory skills and work well with all staff. • Must have reliable transportation, car insurance and a willingness to travel as necessary. • Must have current CPR. • Must have demonstrated clinical skills in the provision of home health care. • Must have at least two years experience in a Medicare certified agency and have excellent understanding of Medicare regulations and documentation requirements. • Must work well with people. • Must have excellent computer skills and enjoy working with both field staff and client records. • Must have ability to establish priorities in all phases of work. Preferred: • A sound knowledge base of home health care needs, regulations, and the complexities of care delivery in current home health practice. Excellent computer skills, home health documentation skills, ability to assume a leadership role in the agency. Should enjoy the day to day operation of the agency and the challenges presented on a daily basis. WORK EXPERIENCE & EDUCATION Minimum Required: • Must be licensed RN in the state of Arizona. • Must have a minimum of two years practice in a leadership role in a Medicare certified home health agency. • Must have excellent documentation and communication skills. • Must have good clinical skills. Preferred: • BSN or Associate degree in nursing with five years experience in home health care with increasing responsibilites during employment. WORKING CONDITIONS: • Working hours are essentially 8-5pm Monday through Friday but some On Call and weekend responsibilities are expected as well. Majority of time will be spent in the office. Works in well-lit, air-conditioned office. Some staff supervision and client visits require travel within Maricopa county.
