Jobing Description
IMPORTANT NOTES:

• This is a part-time position, three days per week, 20 hours per week.
• A Bachelor's of Social Work (BSW) is REQUIRED for this position.
• This position is based at our Phoenix Adult Day Health Services Center in Central Phoenix.

STATEMENT OF WORK:
Provides outreach and determines programmatic eligibility of potential Adult Day Health Care/Outpatient Clinic, Partial Care, Basic participants. Conducts initial psychosocial assessments and reassessments; assists with case records of each participant; facilitates and participates in the careplan conference for each participant enrolled in the program. Performs case work services needed by the program participants; provides supportive services to the families and caregivers of the program participants. Participates in developing community awareness of Adult Day Health Care programs. Assists the participants in the daily program when needed. Performs any other job related duties deemed necessary and/or assigned by the Site Supervisor/Center Director.

DUTIES AND RESPONSIBILITIES
Prescribed:
• Provides outreach to determine programmatic eligibility of potential Adult Day Health Care/Outpatinet Clinic, Partial Care, Basic participants by speaking to individuals and groups, canvassing specific target areas, and by evaluating all referrals. Assesses all participants needs for Center's services as well as programmatic eligibility according to FSAL guidelines. Assesses participant's ability to pay for services and refers those with financial need to the appropriate funding source. Presents a new participant profile to staff upon enrollment of a new participant.
• Completes a psychosocial assessment of each participant, compiling a brief history and assessing the participant's strengths and needs. Facilitates an interdisciplinary careplan conference; creates social service goals with participant and family/caregiver involvement. Records and compiles data as a part of the program's quality and performance review.
• Provides support and guidance to participants. Assists participants and families/caregivers with information and referral to community services. Advocates for participants' rights within the community through networking with other community based agencies.
• Provides supportive services to the families and caregivers of program participants. Shares with the family helpful suggestions and nformation regarding care and support of disabled adults. Provides support and/or offers resources to families and caregivers in crisis situations. Provides information to participants and families/caregivers in topics that are culturally, physically, and age appropriate.
• Takes responsibility for case records by preserving confidentiality of all materials and following established procedures for release of information. Records delivery of service as required by funding source regulations, State Licensure rules, CARF Accreditation standards, and FSAL Programs procedures. Submits forms required to maintain accountability regarding programmatic eligibility for services. Writes progress notes, narrative summaries, and closure summaries at intervals designated in program guidelines.
• Conducts on site tours and enrollment conferences.

Varied:
• May facilitate caregiver support groups, participant support groups, and family education nights.
• May participate in community education programs discussing Adult Day Health Care issues.

Skills / Requirements
KNOWLEDGE, SKILLS AND ABILITIES
Minimum Required
• Needs problem solving, intervention, communication, facilitation, assessment, and case recording skills.
• Must have knowledge of needs of older adults, the aging process, and the needs of adults with disabilities or SMI.
• Must have knowledge of resources available in the community.
• Must have ability to recognize potential problems with participants and/or families/caregivers, and to recommend solutions.
• Must have ability to work under pressure of meeting deadlines.
• Must have ability to adhere to detailed regulations of reporting forms.
• Knowledge of/or experience in the gathering of data, evaluation of data, and external dissemination of data as a part of the ongoing performance improvement process.
• Must be able to work with challenging behaviors expressed by participants, i.e. verbal and physical aggression, wandering, etc.
• Must be able to obtain CPR and First Aid Certification.
• May be requested to obtain a Food Handlers Card.
• May assist with meals according to CACFP guidelines.

Preferred
• Familiarity with government regulations relating to Adult day Health Care programs is desirable.

WORK EXPERIENCE AND EDUCATION
Minimum Required
• Bachelors degree in social work

Preferred
• At least one years paid or volunteer experience in social work.
• Languages other than English, i.e. Spanish, Sign Language, etc.
