Jobing Description
Under the direct supervision of the Area Supervisor, this position is responsible for the supervision, hiring, scheduling, orientation, monitoring and evaluation of four to ten Caregivers/Certified Nursing Assistants (CNA) in an Assisted Group Living (AGL) home. The Team Supervisor will maintain a complete daily coverage schedule (24 hours) for the home. A Team Supervisor will develop and maintain backup and emergency coverage plans for the home in coordination with the Area Supervisor and Director of Nursing. Other responsibilities include, participation in developing and executing participant serve plans, management of the home budget, control of a petty cash fund for home operations, meal planning, planning activity schedules, coordination of participant appointments for services outside the home, home cleanliness, home maintenance, attendance at required training, participation in clinical supervision, conduct in in-service training and other duties as assigned. Team supervisors will normally work in direct care, with the same duties as an Caregivers/CNA's, 28 hours per week. This position will be on-call to home staff at times specified during the work week and may share weekend on-call in rotation with other Team Supervisors.
DUTIES AND RESPONSIBILITIES Prescribed: • Provides support, encouragement and assistance to ILAs in the performance of their duties. Provides direct care, with the same duties as an Independent Living Aide as required. • With assistance from the Area Supervisor, hires and maintains the authorized staff level for one AGL home. • Assists in the development of and conducts required initial orientation and training for all direct care staff. • Participates in management staff meetings. • Meets annual requirements for continuing educations and training. • Assists in problem solving with other Team Supervisors, ILAs and staff members when participant care needs, schedules or staff require it. • Identifies the need for and requests professional consultation from Clinical Management as participant needs dictate. • Monitors and assures compliance with AGL clinical and administrative documentation requirements and other AGL policy and procedure. • Participates in overall AGL policy development and management decision making. • Prepares reports in timely manner. • Accurately communicate the needs of staff, participants and management to and from staff and management. • Provides on-call supervisory coverage on a rotating basis with other management staff. • Maintains accurate staff time records. • Responsible for management of a petty cash account. Prepares current, accurate and complete reports of petty cash expenses as required by AGL Policy and Procedure. • Meet with the Clinical Program Manager or designee on a regular basis for clinical supervision. Follows up on written and oral directions concerning participant care. • Participates in quarterly or special Participant Individual Service Plan staffings and implements strategies directed toward reaching goals and achieving service objective contained in the service plan. • Transports participants to regular and emergency medical or psychiatric appointments and conducts follow up to ensure changes in orders are communicated to appropriate clinical staff. • Informs the Clinical Program Manager, Area Supervisor or Director of Nursing of changes in participant condition, medical orders and due to other contacts as required by AGL Policy and Procedure. • Participate in the Performance and Quality Improvement initiative at the agency and program level by learning about the initiative, offering input, suggesting performance improvements, and performing other PQI duties as requested. Varied: • Under the direction of the Clinical Program Manager, may conduct clinical supervision for direct care personnel, if qualified under Behavioral Health Regulation. • Other duties as assigned Equal Opportunity/Affirmative Action Employer Skills / Requirements
KNOWLEDGE, SKILLS AND ABILITIES
Minimum Required • Ability to listen and to accurately communicate information necessary for quality program operations to superiors, subordinates and participants both orally and in writing. • 21 years or older • Valid AZ Driver's License. Preferred • Current registration with the Arizona State Board as a Certified Nursing Assistant or documentation of completion of training as a Psychiatric Technician or Habilitation Specialist or similar training and education is desirable. • Personal qualities including: Flexibility, tolerance of varied cultures and behavior, sound judgment, good health, reliability, honesty, personal sensitivity, warmth and caring in relationships. WORK EXPERIENCE AND EDUCATION Minimum Required • High School Diploma or GED required. • Experience as a caregiver, preferably with senior population. Preferred • Associate's (two-year) degree from an accredited program in Social Work, Counseling or Human Service related field desired. • Supervisory experience is desirable. WORKING CONDITIONS: • Works within the community traveling to various locations. • Requires driving vehicle during daytime and nightime hours.
