A New Leaf, a non profit agency is seeking a Full Time Client Medical Appointment Transport for their school in Mesa, AZ.
SCHEDULE: Mondays to Thursdays. 7:30 AM to 3:00 PM. Hours might change to meet program needs. PRIMARY FUNCTIONS AND RESPONSIBILITIES: Our school is seeking to fill a Full Time (30 hours) Client Medical Appointment Transport position to schedule and coordinate medical appointments for the Residential Treatment Center programs (RTC). In this position you will also transport RTC clients to medical appointments; communicate follow-up instructions from medical provider (s) to RTC staff, be the liaison between RTCs and medical providers; and assist with coordination with insurance carrier among other things. MINIMUM EDUCATION/SKILLS/REQUIREMENTS:
We are looking for people to promote, model, and teach social skills. The ideal candidate will possess a minimum of a HS/GED Diploma. AA Degree in BH is preferred. Experience working with juvenile population preferred. MS office skills are helpful. A clean driving record is required. Bilingual: Spanish/English a plus. To succeed in this position good verbal and written communications skills are required as well as excellent customer service skills. This position might have other requirements. The following requirements must also be met: 1. Must meet the following driving insurability guidelines: maximum of 1 moving violation in the last 3 yrs in combination w/one at fault accident OR maximum of 2 moving violations in the 3yrs w/no at fault accidents OR maximum of 2 at fault accidents in the last 3 yrs w/no moving violations; All drivers must be licensed for at least 3yrs. 2. Must successfully pass a criminal background check for the purpose of obtaining DPS Class One Fingerprint Clearance Card. A New Leaf will assist you with your fingerprinting at your intake session. If you are currently on probation DPS may deny your Class 1 Clearance and you may be terminated from your employment upon verification of the denial from DPS. The Office of Behavioral Health and Licensing (OBHL) requires that employee is at least 21 years of age. Important Notes
