Jobing Description
We are currently seeking a full-time Case Manager to work with the Unity Project. Responsibilities are to provide or coordinate services (e.g. case management and outreach) to address clients’ individual treatment needs. The Case Manager is expected to cooperate as a team member and provide services to promote positive behavioral change for clients.
Skills / Requirements
Minimum Qualifications:
Associate Degree in BH related field, no experience necessary. OR Associate Degree in non-BH related field and one (1) year behavioral health related work experience (social, human or health related services). Important Notes
Must be 21 years of age or older (licensing requirement).
Must possess a valid AZ drivers license with no major moving violations and no more than two moving violations within the last three years.
