Jobing Description
Quality Care Network (QCN) is incorporated to serve children and their families throughout Maricopa County.

QCN is looking for BILINGUAL/SPANISH SPEAKING Intensive Case Managers that will provide ongoing case management services to clients and families. ICM's will perform out of office case management services and administrative tasks in accordance with agency procedures.

EXAMPLE of Duties and Responsibilities:
• Performance of administrative tasks in conformance with Agency principles
• Demonstrates effective time management skills
• Demonstrates effective engagement and coordination techniques with assigned clients on their caseload as evidenced by supervisor observation
• Demonstrates the ability to recognize and verbalize family strengths, even in difficult times
• Interacts professionally with other stakeholders (Child Protective Services (CPS), probation, Direct Service Providers (DSP), etc.)
• Participates in court hearings for service recipients if/as appropriate
• Continually strives to improve cultural awareness
• Continually strives to improve clinical competency (trainings, reading, school, etc.)
• Maintains a general knowledge of services and supports available in the community, both from inside and outside of the agency for which the case manager works
• Demonstrates the ability to monitor the delivery of services to ensure they are addressing service plan goals
• Demonstrates quality written documentation that reflects correct spelling and grammar, appropriate use of behavioral health acronyms, appropriate use of therapeutic terminology
• Demonstrates ability to create a written document that accurately captures the strengths, needs and culture of the family
• Collaborates with Clinical Liaison for clinical oversight and documents this interaction within standard timeframes
• Completion of Service Plan with collaboration of family, team members and Clinical Liaison
• Demonstrates team work and shared leadership
• Demonstrates facilitation skills including the ability to establish an agenda, setting and maintaining a strength based meeting environment, staying within timeframes, giving all team participants the opportunity to contribute, holding people accountable for their commitments and the ability to assist a team in developing a consensus
• Meets monthly productivity standards each month as determined by supervisor
• Maintains a current AZ Drivers License including ability to drive and current auto insurance








Skills / Requirements
Bachelor degree in a behavioral health related field or;

-Bachelor degree in any discipline with at least one year of full-time direct behavioral health work experience or;

-Four years of direct behavioral health work experience with an Associative Arts degree, High School diploma or GED.
