Intermountain Centers is a private, not-for-profit service organization, which provides residential and home-based services to a variety of clinical populations. Intermountain programs are designed to develop interpersonal and social skills, self-sufficiency and self-management skills, academic and vocational skills, community awareness and responsibility and constructive future planning. We are currently recruiting for an Intake/Assessment Case Manager for our Birth to 10 yr. program team.
The Intake/Assessment Case Manager is responsible for the initial assessment of needs and coordination of treatment services for program participants following their referral to Intermountain Centers for out-patient behavioral health services. The Intake/Assessment Case Manager acts as the primary point of contact for all Child and Family Team (CFT) participants to facilitate meetings and follow-up recommendations, to ensure continuity of care and service delivery, and to educate team members about the CFT process, until child/family are assigned to an ongoing worker Skills / Requirements
PRINCIPLE DUTIES AND RESPONSIBILITIES:
• Act as the first point of contact to children/families referred to ICHD by Pantano Network • Meet with children/families to begin the process of assessing the family’s strengths and needs through the creation of a Strengths, Needs and Cultural Discovery (SNCD), Service Plan and Safety Plan; and level of service intensity, using identified tools. • Begin the process of developing a comprehensive Child and Family Team, to include identification of natural and community supports and professional involvement (when needed); schedule and coordinate the first Child and Family Team meeting within the required time frames • Facilitate initial CFT meetings to educate partners in the levels of care, service availability (both formal and informal) and CFT process. • Schedule and coordinate transition CFT meetings with other In-Home Services or subcontracting staff • Provide referrals to outside providers and community resources in accordance with HIPPA policies: coordinate transition for services. • Develop and monitor initial treatment plans with clients and participating agencies • Attend and participate in team meetings; clinical staffings as needed; hospital discharge planning meetings; court hearings; and other relevant meetings to meet the treatment needs of program participants • Assist with discharge planning for children leaving residential treatment or hospitals, and being referred to ICHD for out-patient services • Maintain client records in accordance with agency and funding source requirements • Provide complete documentation and reporting of client services in accordance with agency and funding source requirements. KNOWLEDGE, SKILLS AND ABILITIES: • Knowledge of managed care systems and AHCCCS • Knowledge of behavioral health services planning process and timeframes • General knowledge of services and supports available in the community • Knowledge of and the ability to seek culturally sensitive services. • Knowledge of the CFT process; the Arizona Vision and the 12 Principles • Demonstrated basic computer literacy, to include familiarity with Microsoft Word application, and data entry • Organizational abilities to maintain communication, coordination, and connections with all CFT participants. • Demonstrated ability to communicate effectively, orally and in writing • Demonstrated ability to follow written and/or verbal requests and instructions • Knowledge of, and ability to utilize, effective engagement and coordination techniques • Ability to work effectively as a member of an integrated team • Bi-lingual skills preferred • Ability to handle a moderate to high level of stress • Ability to work a flexible schedule to include some evenings and weekends. • Ability to lift up to 25 lbs. QUALIFICATIONS: • Master’s degree or a bachelor’s degree in a field related to behavioral health; a bachelor’s degree and at least one year of full-time behavioral health work experience; or an associate’s degree and at least two years of full-time behavioral health work experience; Child Development education and/or experience preferred. • Minimum 21 years of age • Possess a valid Arizona Driver’s License and 39-month driving record that meets insurance requirements; • Legal record void of any felony; • All employees must be fingerprinted Important Notes
Bi-lingual differential of $1,500 for fluency in Spanish, based on a standard test criteria
Must be able to provide own reliable transportation to and from clients’ homes and/or other relevant organizations; business mileage is reimbursed May be required to work in other organization’s facilities and in client’s homes.
