Jobing Description
Looking for a Social Service Director to join our team.
As part of the admission process, meet with the resident and family to review policies on advanced directives, residents rights and discrimination.
Conduct and document a social service evaluation, including identification of resident problems and needs.
Provides and arranges for social work services as indicated by residents and family needs.
Assist residents with financial planning as needed.
Keeps residents family or responsible party informed of residents problems, personal needs, transfers and level of care change.
Maintains a current list of community resources to facilitate referrals.
Working knowledge and ability to apply professional standers of social work practice in job situations. Able to establish criteria to assure that services provided meet established standards of quality.
Demonstrates satisfactory level of interpersonal skills to interact with residents, families, community and interdisciplinary staff.
Demonstrates assessment skills sufficient to evaluate residents behavior, to collect data to evaluate psycho-social needs, risk factors for psycho-social deterioration and residents responses to interventions. Able to interpret and explain residents behavior to staff in a way that fosters understanding, facilitates treatment and respect for resident rights and minimizes use of chemical and physical restraints.
Able to implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance quality of life.
Complete assessments, MDS, care plans and gather information to meet residents functional levels and reflect needs and interests of each resident.
Participates in development of the department budget provides relevant financial information to Administrator regarding department financial needs and status.
Actively participates in long term care survey process and works with administrator to develop responses to survey report as needed.
Ability to comply with facility polices and procedures for workplace safety, including infection control procedures, application of universal precautions for blood borne pathogens, use of personal protective equipment and handling of hazardous materials.
Assure participation in relevant in-service training sessions.
Attend and participates in facility meetings.
All other duties as assigned.

Skills / Requirements
Must have a Bachelors Degree
