Jobing Description
Responsible for providing in-home family support services to teen parents and their families.
These services include conducting family centered assessments and service planning with families with children 0-5 years of age, providing intervention services, and providing referral, coordination, and advocacy for services needed to assist families in working towards their goals. Responsible to Program Director. RESPONSIBILITIES: Initiates contact with referred families and actively works to engage families in the program. Conducts family centered assessments. Facilitates the family centered service planning process to establish goals with each family. Conducts home visits at times which are convenient for the family. This may include evenings and weekends. Provides case management at a level of intensity that meets the specific needs of each family. Provides parenting/life skills guidance that meets the needs of each family. Maintains case records in accordance with agency and program policies, procedures, and requirements. Maintains positive working relationships with collaborative partners, and others within the community. Attends weekly supervision to review cases. Attends and participates in all scheduled conferences, seminars, in-service training and program meetings as requested by the Program Director. Participates in Continuous Quality Improvement activities. Performs all other duties as assigned. Skills / Requirements
REQUIREMENTS:
Knowledge and skills in Family Centered Practice. Knowledge of community resources. Knowledge of and sensitivity to cultural differences. Ability to communicate effectively, both orally and in writing, with a variety of populations including families and other social service professionals. Ability to work effectively as part of a team. MINIMUM QUALIFICATIONS: Completion of a Bachelors degree in Social Work or related field. Reliable transportation, current Arizona Drivers License, and automobile liability insurance required.
