TITLE: Case Manager BHT
HOURS OF WORK: Full Time, hours vary
LOCATION: Residential or Out Patient

MISSION:
Improving the lives of Native American individuals and families through culturally appropriate behavioral health, affordable housing, and community development services

POSITION SUMMARY:
Under the direct supervision of the Clinical Supervisor, provides intake, orientation and case management services for clients participating in Native American Connections programs.

RESPONSIBILITIES:
• Complete initial orientation and admission of incoming clients including data collection, signatures, setting up files, obtain releases, data entry
• Assist with inventory and distribution of client supplies, and the return of those supplies at discharge.
• Emergency follow-up on any communicable diseases noted during intake.
• Maintain approved client files documentation procedures
• Engage client in services, explaining NAC services
• Assist with or facilitate skill groups; coach individuals and/or groups in life skills development
• Assist with weekly recreation activities and transportation as needed
• Provide referrals for emergency stabilization needs, basic needs and supplied, housing, employment, job Training, etc.
• Provides support for work force development, helping with resumes, interviews as indicated by client need.
• Develop individual and client cultural competencies
• Assist in obtaining benefits such as with applications for food stamps, GA, AHCCCS.
• Coordination of GA, check cashing and budgeting according to NAC policy.

Skills / Requirements
EDUCATIONAL REQUIREMENT:
• Master’s Degree or Bachelor’s Degree in behavioral health field with 2 years experience or Associates of Arts Degree in behavioral health field and 4 years experience (Behavioral Health Technician)

WORK EXPERIENCE/SKILLS REQUIREMENT:
• Understand the dynamics of chemical dependency and demonstrate the ability to integrate the knowledge for practical application.
• Know the economic, educational and social problems of Native Americans and referral sources available.
• Excellent communication skills – written and oral
• Possess and maintain a valid Arizona drivers license
• MS Office skills
