We currently have three offices in the Phoenix metro area and need staff members that are willing to grow with the company.

All Valley is opening a new skilled office and needs a few per diem nurses and aides for the Central Phoenix and Easy Valley areas.

Job Description -

- Skilled service only, not Medicare. No Oasis forms!
- Assess and evaluate patients on admission; regularly re-evaluate patient nursing needs. To initiate, write and revise the plan of nursing care.
- Initiate and apply appropriate preventative, therapeutic and rehabilitative techniques and to maintain open communication regarding patient care with other members of the health care team.
- Complete, maintains and submit accurate and relevant clinical notes referring to the patient's condition and response to treatment.

Current license/certification with the state of Arizona.
Current CPR,TB, and Fingerprint card
Self-directed, flexible and able to work with little supervision.
Good observation skills, good oral and written communication skills.
Home Health experience required, within the last year.
RN - Resource Utilization / Case Management experience required.
