Summary:
Through delegation of tasks by the assigned members? Clinical Liaison, facilitates assigned members? adult recovery teams in securing and managing behavioral health services that aid in the member?s recovery from mental illness and/or addiction disorders. Essential Duties and Responsibilities: 1. Provides supportive services to enhance treatment goals and effectiveness. 2. Assists members in securing a variety of social services, including but not limited to, relapse prevention services, prenatal care, medical care, food stamps, general assistance and child-care. 3. Communicates and coordinates care with the members family, behavioral health and general medical and dental health care providers, community resources, ad other involved supports including educational, social judicial, community and other State agencies. 4. Provides outreach and follow-up of crisis contacts and missed appointments. 5. Participates in staffings, case conferences or other meetings, as appropriate. 6. Transports members to and from appointments when appropriate. 7. Provides community and home-based services to members, when appropriate. 8. Monitors level and intensity of services contained in the members? individual service plans. 9. Maintains member records in accordance with agency and funding source requirements. 10. Maintains regular contact with treatment staff to inform and be informed of members progress and makes recommendations for continuing care. 11. Meets with service providers to review utilization and monitor member progress towards program goals. 12. Provides member advocacy, as needed, to ensure delivery of required services. 13. Provides community outreach and education regarding program services. 14. Provides crisis intervention to include location of appropriate providers of service. 15. When appropriate, develops transitional service plan with members, prior to reentry into the community. 16. Performs other related duties as directed. Education and/or Experience: A combination of one of the following education and work experience are required: Associate degree in a behavioral health field AND 3 years behavioral health experience; OR, Associate degree in a non-behavioral health field AND 30 semester hours in behavioral health education, plus 4 years behavioral health experience; OR, Bachelor's degree in psychology, counseling or related behavioral health field AND 1 year related experience; OR, Bachelor's degree in a non-behavioral health field AND 30 semester hours in behavioral health education, plus 3 years behavioral health experience; OR, Master's degree in behavioral health field; AND 1 year of related experience; OR,Master's degree in a non-behavioral health field AND 30 semester hours in behavioral health education, plus 1 year behavioral health experience. Certificates/Licenses/Registrations: Must provide own transportation, a current Arizona Driver's License and automobile liability insurance. Skills / Requirements
Knowledge/Skills/Abilities:
1. Demonstrates clinical competency in accordance with standards for the position. 2. Demonstrates competencies in accordance with CODAC standards and metrics. 3. Extensive knowledge of community resources. 4. Extensive knowledge of behavioral health challenges which include knowledge of the impacts of mental illness, domestic violence, and the dynamics of addictive behavior. 5. Ability to communicate effectively both orally and in writing. 6. Ability to interact effectively with other service providers. 7. Familiarity with various ethnic and cultural groups. 8. Intermediate to advanced computer skills using MS Office products, Word, Excel, Access, etc., importing/exporting data to/from applications. Important Notes
