Pharmacist- Solid Organ Transplant
Date: Oct 8, 2009 Location: Phoenix, AZ, US Job Posting: 7222 This ambulatory Pharmacist position will support the Kidney, Pancreas and Liver programs in addition to supporting CMS mandates related to drug interactions or complications. This incumbent will also perform the following tasks: participate in Kidney, Pancreas and Liver Transplant outpatient clinics, support transition of outpatient prescriptions to the Specialty Pharmacy Work Group, attend and document at daily inpatient rounds on new Kidney, Pancreas and Liver Transplant patients, coordinate current patient issues and therapy recommendations with inpatient pharmacy staff, provide oversight for medication education, support Research activities of the Kidney, Pancreas and Liver teams as requested, and attend weekly selection conferences for Kidney, Pancreas and Liver Transplant Teams. Solid Organ Transplant experience highly desired. Department: Transplant Center Qualifications: Responsibilities include distribution, drug therapy monitoring, education and supervisory functions. Must collaborate with others, organize well, accomplish tasks, solve problems and communicate effectively. The individual must demonstrate knowledge of the principles of growth and development of the life span. The ability to assess data reflective of the patient's status and appropriately interpret information relative to the patient's age-specific needs is required. Minimum Education and/or Experience Required: Graduate of an accredited College of Pharmacy. Additional Experience: Hospital experience desirable. Licensure/Certification Required: Current license to practice pharmacy in the State of Arizona.
