Qualifications:
Be a Doctor of Medicine or Osteopathy.
Successfully completed an internship and residency in an accredited U.S> Training Program ACGME approved residency and be Board Certified in their respective specialty.
Have and Maintain an active, valid, unrestricted current medical license in a U.S. Jurisdiction. License must correspond to DEA registration number.
