Posted Date: 10/9/2009 Shift: Varied Category: Pharmacists - Positions Department: Pharmacy Facility: Banner Thunderbird Medical Center Position Type: PerDiem-CA Shift Hours: Varied Hours Per Week: Varied Overview: Under the guidance of the Staff Pharmacist, the Pharmacist Intern is responsible for the delivery of pharmaceutical care to individuals within the medication use process (selecting, prescribing, dispensing, administration and monitoring of medications). This position performs comprehensive assessments and analysis in collaboration with integrated team members in meeting the healthcare and educational needs of the patient. The Staff Pharmacist oversees the activities of the pharmacy technicians, coordinating workflow and checking all medications prepared for patients. Additionally, the Staff Pharmacist is accountable for on-going evaluation and documentation of all components of pharmaceutical care delivered to patients of Banner Health System. The Staff Pharmacist must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients being served while maintaining the ideals of Service Excellence. The Staff Pharmacist must also possess the ability to assess data reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements and provide pharmaceutical care based on those needs. Responsibilities: 1. Reviews medication orders and evaluates their appropriateness and completeness with respect to patient data including age, height, weight, diagnosis, laboratory values and other pertinent patient information. Contacts the prescriber with suggestions for improvement and resolves problems with the medication orders as needed. Documents resolutions in the appropriate manner. Complies with directives of the Pharmacy and Therapeutics Committee as it relates to formulary management. 2. Utilizes clinical pharmacy skills in assessing and formulating recommendations to physicians ensuring quality and economical drug therapy and positive patient outcomes. Performs medication assessments with special consideration given to medication safety and needs of the specific patient age groups served. Participates in and supports decentralized pharmacist activities, when and where applicable and appropriate. 3. Enters medication orders into the computer system in an accurate and timely manner. Assures the patient medication profile presents no significant problems in regard to drug interactions, incompatibilities, therapeutic duplications or patient safety. Dispenses medications and IV solutions in an accurate and timely manner. Dispenses medications in conformance with state and federal statutes and hospital procedures. 4. Supervises and directs work of pharmacy technicians and interns. Coordinates medication preparation and distribution work of the pharmacy technicians and ensures accuracy prior to dispensing medications for distribution. 5. Provides accurate, appropriate and timely information to physicians, nurses and others regarding medication therapy, medication systems, drug product guidelines, law and regulations, medication formulary and other topics related to the medication use system. Provides needed education to patients regarding their medications. 6. Ensures accurate records are being maintained for controlled substances. Adheres to local, state and federal laws regarding the profession of pharmacy practice. Skills / Requirements
Requires (or eligible) pharmacy licensure in the State of Arizona and a degree in Pharmacy (advanced degree preferred) from an accredited College of Pharmacy. Prefer hospital pharmacy experience. Must possess good organizational, problem solving and computer skills. Must possess good written and verbal communication skills for interfacing with all levels of Banner Health System staff, physicians, patients and other contacts. Must be able to use reference tools to evaluate patient care and answer questions posed by customers. Must have knowledge of computer software, particularly pharmacy system software, Microsoft applications and Pyxis.
