Job: Outreach Case Manager

Central Arizona Shelter Services (CASS), the largest provider of homeless shelter and services to single adults in the Valley of the Sun, is looking for an Outreach Case Manager for our Mens Outreach Shelter in the downtown Phoenix area. The mission of CASS is to empower men, women, and children with diverse needs to end their homelessness by providing shelter and supportive services. CASS seeks to hire a full-time Outreach Case Manager to engage Mens Outreach Shelter (MOS) clients in services and assist clients in securing services to achieve stable housing in a self-sufficient environment by providing high-quality outreach case management services.

Essential Functions:

Engage homeless individuals residing in MOS and develop an effective service plan for clients to access services to progress towards self sufficiency.

Research database and files for background information on returning clients.

Conduct an intake discussion with client, obtaining as much relevant information as possible.

Assess client needs for developing individual service plan and goals.

Review service plan with client, providing resource information, referrals, and direction.

Set expectations with client of agency guidelines and compliance with case plan.

Support client in achieving case plan goals.

Work closely with outreach services, emergency shelter, collaborative partners and Human Services Campus supportive services to link clients and coordinate services to achieve service plan goals.

Conduct follow-up discussions and provide walk-in assistance as needed.

Determine and administer consequences for non-compliance with service plan goals.

Provides information on available resources and makes client referrals to appropriate programs and agencies.

Act as an advocate for the client, as appropriate, with other providers.

Establish and maintain referral resources in the community.

Maintain complete and organized client files.

Enter relevant data in client database system on a timely basis.

Compile data and produce reports for analysis and assessment of program effectiveness.

Maintain program quality assurance standards.

Provide “success stories” and other positive client progress information to development team.

Required Qualifications:

High school diploma or equivalent plus a minimum of 1 year directly related experience.

Experience with homeless or human services.

Proficient in Microsoft Office Suite (Word, Excel etc.).

Demonstrated communications skills, both oral and written.

Demonstrated problem solving and critical thinking skills.

Strong interpersonal skills.

Ability to type 25-35 words per minute.

Possess a valid Arizona driver license and reliable transportation.

Preferred Qualifications:

Bachelor degree in social or behavioral science or related field.

Experience working with databases.

Bilingual in English and Spanish.
