Job: Care Manager

This position will be responsible for providing individualized assessments, planning, coordination and monitoring of the multiple services needs of our clients, their families and care givers in all stages of their life cycle.

Responsibilities include but are not limited to:

Intake and assessment of individual need
Information and referral when appropriate
Development of individualized assessment plan with input from the individual, their family and other appropriate providers
Implementation and coordination of service plan
Monitoring of client needs and plan adjustments as needs change

Qualifications:

Completion of a Bachelor's degree in nursing, counseling, social work or a related field, masters degree preferred.
Licensed fiduciary preferred
Knowledge of community resources that may be utilized on client behalf
Skill in establishing and maintaining control over case plans, schedules, timetables and priorities
