How do we do this?  Through dedication to our staff.

* We serve Mariocpa and Pinal Counties, where you can create a manageable “care territory” that you agree to see patients within.

* We have streamlined forms and paperwork including admission documents, OASIS, and clinical notes to minimize the time spent unrelated to direct patient care.

* Our office staff acquires all insurance authorizations for managed care cases leaving you, the clinician, to focus on patient care.

* We are also dedicated to providing our staff with the best tools and latest advances in technology to be able to perform their jobs better and more effectively.  Horizon incorporates the advances of telemedicine in our standard of care by using Honeywell HomMed’s Genesis and Sentry monitors.  This allows our nurses to be updated on a daily basis with patients’ vital signs (including BP, SP02, HR, temperature, and weight) in addition to their scheduled visits.  Using this information, our nurses gain the ability to more proactively manage a clients care.
