Jobing Description

The Family Aide, under the supervision of the Supervisor, assists staff in performing care coordination services, participates in staffings, court hearings and other designated meetings. Assists in providing care coordination services and actively participates in activities of the care coordination team. Assists members/families to access needed services, benefits and entitlements which may include providing transportation. Assists in maintaining appropriate member records, service delivery forms and evaluation forms as necessary. Assists in obtaining necessary record, reports from providers and/or other agencies providing services to members. Maintains complete, timely, accurate documentation as appropriate for all services/activities performed. Assists in preparation of referral packets. Assists in tasks associated with Project evaluation process. Assists in quality assurance activities, particularly as they pertain to member records. Performs other duties as assigned.

Skills / Requirements

Minimum Education/Experience Qualifications:



One year experience in a behavioral health setting is required. Must have the ability to operate a personal computer keyboard and telephonic communications equipment. Must have the ability to read the screen of a computer monitor and the ability to read hand written and printed documents and records. Must have the ability to perform analysis of moderate complexity. Must have the ability to move around an office environment. Must have the ability to communicate ideas of moderate complexity in writing.

Important Notes

Must have current level 1 fingerprint clearance and be able to pass the background check. Clear MVD required. TB test, First Aide and CPR training are required.
